INTERNAL ONLY – eBlast and Survey Request Form
This internal ONLY document is designed to allow for an efficient method of requesting an eBlast or survey campaign.
The form will require complete information about the eBlast or survey including the requesting department, dates, type, customer (if requested), product information, requirements, and a spreadsheet list of contacts to use.
In addition, once submitted, the form will be sent electronically to Marketing and Sales Management, and the requester will receive a conformation email showing the submitted information.
